146 - Job Coach Provides on the job training (off-site locations in New Haven, Waterbury & Fairfield County) to ensure that each person's individual vocational assessment or coaching goals are met. Traveling is a requirement as this position is at various sites. Observes the individual and document their’ behaviors and process in the vocational assessment and/or case notes. Observe an individual's work behaviors and progress, which will be documented, in the vocational notes and indicated on the assessment report. Assists in the completion of vocational assessments. Able to work a flexible schedule, which may include occasional evenings and weekends.
