Position Summary

The Care Progression Clinical Supervisor is an expert-level clinician who oversees care progression and care coordination across a defined geography (typically a ‘floor’ comprised of multiple patient care units). Reporting directly to the Director of Care Progression and Transitions Management, the Clinical Supervisor is a first-line supervisory position responsible for supervising a team of unit-based Care Managers and Transition Coordinators. The role ensures care is delivered at the right place, at the right time, for the appropriate duration, with a focus on safe and smooth transitions to the next level of care.

The Clinical Supervisor also leads daily care briefings and ensures collaboration among interprofessional team members to optimize care progression. This individual serves as a coach, mentor, and role model on individual units for bedside RNs and other interprofessional staff. They identify barriers to care progression and care coordination, escalate to appropriate parties, and work towards resolution. The incumbent oversees the assessment, planning, implementation, and evaluation of all patient-centered care coordination activities, develops and enforces standard work, and conducts performance evaluations for their team. They must also be cross-trained to act as a care manager/transition coordinator as needed.

The success of the Clinical Supervisor depends on their ability to communicate positively and professionally with patients, families, physicians, and staff to optimize workflow and facilitate problem identification and resolution. Essential Functions and Responsibilities of the Job

1. Leadership and Direct Supervision: Supervises the care coordination team, managing accountability systems, completing staff evaluations, and assisting with staff training. Provides performance improvement guidance and serves as a coach/mentor for staff, nurses, and interprofessional team members.

2. Continuum of Care: Acts as a clinical subject matter expert in care progression, supervising care coordination and transition planning to ensure continuity of care. Demonstrates financial stewardship of hospital resources through utilization and length of stay indicators.

3. Partnership and Collaboration: Collaborates with Nursing Operations Leadership to ensure quality patient care and nursing excellence, ensuring seamless patient flow with Patient Throughput Leadership.

4. Advocacy and Empowerment: Advocates for appropriate care placement and timing, ensuring respect for patient and family rights and safety. Supports staff in removing barriers to safe patient discharge and empowers patient self-management.

5. Physician Liaison: Serves as a liaison with physicians/providers to ensure progress on patient care plans, escalating issues to the Medical Director/Physician advisor as needed.

6. Quality Improvement: Adopts a Lean mindset and the PDSA model for performance improvement, leading Quality Improvement initiatives.

7. Information Management: Utilizes software systems and electronic devices according to standard work and policies.

8. Staff Engagement: Inspires high performance, fosters employee engagement, and creates a supportive work environment that enhances service excellence and the Patient and Family Experience.

  • Placement in the pay range is based on multiple factors including relevant experience and qualifications.
