Location: Deltona, Florida

Shift: Full-Time

Salary: Up to $40 per hour

Job Description:

Bedrock Rehabilitation and Nursing Center at West Volusia is seeking a knowledgeable and dedicated RN MDS Coordinator to join our clinical team. The MDS Coordinator plays a key role in the assessment and care planning process for residents, ensuring accuracy, timeliness, and compliance with all federal and state regulations.

Key Responsibilities:

• Complete and coordinate MDS assessments in accordance with regulatory requirements.
• Review and update resident care plans to reflect individualized needs and goals.
• Maintain accurate documentation, including forms, reports, and evaluations.
• Lead quality assurance programs related to MDS and care planning.
• Work closely with the Discharge Planning Coordinator to ensure smooth transitions of care.
• Participate in facility inspections and surveys.
• Analyze Quality Indicators (QIs) and Quality Measures (QMs) to identify and address trends.
• Ensure timely electronic transmission of assessments and communicate any delays to leadership.
• Collaborate with interdisciplinary teams and families to ensure continuity of care.
• Assist with resident evaluations, therapy coordination, and restorative care planning.
  
  
  

Requirements:

• Active RN license in the state of Florida.
• Minimum of 2 years’ experience in long-term care or skilled nursing preferred.
• Solid understanding of MDS 3.0, care planning, and regulatory guidelines.
• Strong communication, organizational, and leadership skills.
