Select Clinical Home Health Intake Coordinator (LPN)

Select Home Health Services is a central Indiana, clinician based, locally owned and operated, medical home health care agency providing you with skilled nursing and therapy services.

The Home Health Intake Coordinator completes the full intake process, including but not limited to; inputting the patient in HCHB, confirming the following Physician, and completing and verifying the F2F.

The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

• Efficiently process incoming referrals for all branches.
• Understands the regulations and requirements for admission to Home Health.
• Communicates with the following physician and patient when needed.
• Communicate with Sales and Marketing staff about accepted and declined referrals.
• Coordinates and confirms a valid Face to Face visit and attestation form for each referral.
• Promote compliance with incoming documentation.
• Monitor current admission medicare to private insurance percentages.
• Runs and maintains intake reports within HCHB.
• Maintains a referral tracking sheet.
• Demonstrate a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
• Performs job in compliance with agency policies and procedures as well as community and professional standards.
• Communicates necessary information to the supervisor and management team to ensure coordination of services and activities.
• Current LPN licensure in state of Indiana
• Business and professional setting experience
• Healthcare experience perferred
• Excellent written and verbal communication skills
• Speak, read, write and comprehend English
• Ability to handle multiple tasks simultaneously and meet deadlines
• Excellent organizational and time management skills
