Job Summary: The Traveling HRA Nurse Practitioner is responsible for performing comprehensive, non-invasive physical assessments in patients' place of residence (Long Term Care, Assisted Living, Independent Living) This role involves full time-travel within a region, with potential assignments in other areas based on organizational needs. The NP will complete an average of 20-30 assessments per week, ensuring accurate documentation and providing basic patient education on wellness and preventive healthcare measures.

Responsibilities Include
- Travel to and from patients' places of residence throughout assigned regions.
- Perform comprehensive health risk assessments, including reviewing medical histories and conducting physical examinations.
- Ensure accurate and timely documentation of assessments.
- Conduct medication reconciliation to ensure patient safety and compliance.
- Provide basic patient education on wellness and preventive healthcare measures.
- Collaborate with Curana and facility teams to ensure continuity of care.
- Maintain confidentiality and comply with all regulatory requirements.

- Active Nurse Practitioner Licensure in two or more states where Curana is located
- Unrestricted licensure in (insert markets) with ability to obtain additional licenses within first 30 days

- Former I-SNP or HRA vendor experience preferred
