POSITION PURPOSE/SUMMARY: Responsible for the management and delivery of clinical care to patients and families in the HSJ service areas in accordance with Federal and State standards and laws. Directly supervises and schedules the assignments of the RN nursing staff. Under the direction of the Chief Clinical Officer, the Team Leader handles phone calls from RN Case Managers, assists with care coordination, backs up the LVN Supervisor, and triages needs appropriately. The Team Leader is responsible for the coordination and supervision of assigned staff and ensuring compliance of patient visit schedules, quality assurance activities, and coordination of care in the absence of assigned staff.
Key Responsibilities
- Manages assigned staff work schedules, including approval of PTO and overtime, and adjusts work schedules to meet patient care needs. Implements Low Census under the direction of the DHCS. Notifies the receptionist of assigned staffing for the day.
- Ensures case management is carried out in accordance with hospice policies.
- Updates staff on changes to policies and procedures as directed by the CCO.
- Collaborates with all Team Leads to arrange patient services such as CADD pumps and palliative modalities as ordered by the Physician and requested by the RNCM.
- Responsible for notifying the CCO and/or Administrator on-call of any unusual occurrences or adverse events, facilitating immediate follow-up as appropriate.
- Serves as Clinical Supervisor On-call on a rotating schedule.
- Ensures compliance with the Hospice program plan by assuring quality service, appropriate utilization of resources, and safety for employees and patients/families.
- Maintains up-to-date patient records in accordance with Medicare, MediCal, State Home Health Agency, and Joint Commission standards.
- Completes initial nursing assessments and identifies the needs of the patient within regulatory requirements.
- Works with social services to identify the emotional, psychological, and spiritual needs of the patient.
- Collaborates with physicians to determine the frequency of nursing visits and the need for changes in the level of care.
- Provides support to families at the time of the patient’s death, coordinating services with physician, mortuary, DME, and suppliers.
- Assists families in transitioning from caregiving to bereavement.
- Functions as a patient advocate, seeking changes in optimal care settings.
- Works with pharmacy, therapists, DME suppliers, and other healthcare providers to ensure coordination of patient care.
Job Qualifications
- Currently licensed to practice professional nursing in California.
- BSN degree preferred.
- At least 3 years dealing with acute care patients; home care or hospice experience preferred.
- Supervisory experience.
- Ability to set priorities, organize, and work independently.
- Ability to learn and use the Netsmart computer software program.
- Effective communication skills.
- Knowledge of hospice care and commitment to its philosophy and goals.
- Ability to make independent clinical decisions.
- Knowledge of medications, their indications, dosage ranges, side effects, and potential toxicity.
- CPR training.
