Job Summary:

To assess and evaluate all resident’s risk for skin breakdown, ensuring that all necessary measures to prevent and heal skin breakdown are implemented and in use. Provide quality care services to residents requiring skin and wound treatment modalities, in accordance with physician’s orders.

Duties:

The following description of duties is intended to outline the essential responsibilities of the person in this position. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is a logical assignment to this position. All duties performed are expected to promote and support compliance with all applicable laws, regulations, policies, procedures and the code of conduct.

  • Will follow and assist in the enforcement and implementation of facility policies, standards, protocols, and procedures regarding the care and/or any other associated types of care related to the maintenance of the resident’s integumentary system.
  • Will follow the facility’s skin care protocols by assessing all residents on admission and periodically in accordance with their risk for breakdown. Assessments will be documented in accordance with facility policy.
  • Will initiate contact with physicians and resident’s families on need for and progress of treatments.
  • Will assess need for, then assign, implement and monitor interventions designed to prevent skin breakdown or promote wound healing such as but not limited to; turning schedules, pressure reducing/relieving devices and/or nutritional support.
  • Will actively participate in the facility’s quality assessment/performance improvement (QAPIC) activities including but not limited to the following:
  • Conduct and document random weekly audits to monitor staff compliance with at a minimum turning and positioning schedules and pressure relieving device usage.
  • Check treatment sheets at least every Monday and Thursday to verify that they are completed appropriately, notifying the Director of Nursing of any problems that cannot be resolved with individual nurses.
  • Complete and provide a weekly pressure ulcer wound report to the Director of Nursing, Administrator, MDS and Care Plan Assessment Coordinators, Dietary Manager and Facility Nurse Consultant.
  • Make daily assigned rounds to verify that resident care is being provided in a safe and dignified manner. Follow-up on identified problems to ensure corrections are made.
  • Monitor daily shower schedule and ensure that showers and body checks are being completed on all residents in accordance with facility protocol.
  • Ensure, through periodic observation that all treatments in the facility are carried out in accordance with physician’s orders and manufacturer’s recommendations.
  • Submit, for review by the QAPI Committee, statistics and trend analysis data on pressure ulcers.
  • Complete a thorough assessment and, at a minimum, weekly documentation on wounds including but not limited to; stasis and pressure ulcers. Documentation may be required more often and should follow the facility’s prescribed guidelines.
  • Participate in recapping of physician’s orders monthly, as assigned.
  • Make assigned rounds daily and see that necessary corrective measures are completed.
  • Attend and actively participate in Medicare and Care Plan meetings.
  • When necessary, provide resident and family education on wound treatments as dictated by the discharge planning process.
  • Administer and perform medical procedures and treatments as ordered by physician(s) or in response to emergency situations as needed.
  • May be required to work the floor as a staff nurse in the event of a nursing shortage.
  • Rotate on-call duties and weekend Administrative coverage as assigned by the Administrator and/or Director of Nursing.
