Responsibilities
Responsibilities
- Perform patient assessments and monitor vital signs and fluid status.
- Administer medications and treatments according to prescribed care plans.
- Educate patients and caregivers on heart failure and kidney disease management.
- Assist with procedures such as central line care, dialysis support, and IV therapy.
- Coordinate care transitions from inpatient to outpatient settings.
- Document patient care activities accurately in electronic health records (EHR).
- Support the care team in quality improvement and patient safety initiatives.
- Active RN/NP license
- ADN or BSN degree (BSN preferred)
- BLS/ACLS Certification
- 1-2 years of experience in cardiology, nephrology, or critical care preferred.
- Strong patient education and care coordination skills.
