Summary: The Behavioral Health Client Navigator works in collaboration and partnership with Behavioral Health (BH) clients coordinating their care with other providers, family members and community resources in a team approach and as a patient advocate. Goals of this program include but are not limited to, enhancing health, mental health, and wellbeing, improving client satisfaction, connecting clients to resources, and acting as an advocate and client navigator, increasing utilization of psycho-social programs as well as primary/preventative care through enhanced care coordination.

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

  • Establishes network of community resources to fill the gaps in care for BH clients.
  • Meets with BH Program Management and staff in person and during clinical rounds meetings to determine client needs.
  • Helps connect clients to services and resources for basic needs, transportation, benefits, higher level of care, etc.
  • Maintains enhanced care management services with assigned case load of enrolled patients.
  • Assists with maintaining accurate and updated care coordination records, including electronic records for care plans, outreach activities, and correspondence with internal and external resources generating reporting analysis.
  • Codifies data, generates data reports, analyzes reports and takes corrective actions to improve desired program outcome.
  • Other duties as assigned.


Qualifications:

  • Experience working in a fast-paced behavioral health facility.
  • Demonstrated understanding of case management and care coordination.
  • Demonstrated knowledge of EHR systems, preferably Ochin/Epic.
  • Excellent writing and verbal skills.
  • Preferred: Fluency in a second language


Education and/or Experience:

  • Required:
  • Minimum 3 years’ experience in a clinical setting.
  • Bachelor’s Degree
  • Preferred: degree in the mental health care field or similar

Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
