Bachelor's degree in Social Work or similar professional qualifications; Current Social Work license as regulated by state law. Two years of social work supervised experience. Working directly with geriatric clients in a health care setting preferred.

Pay: $65,000.00 - $80,000.00 per year

Responsibilities:
- Conduct diagnostic evaluations and assessments to determine the needs of patients
- Develop and implement treatment plans based on patient assessments and goals
- Provide individual and group therapy sessions using evidence-based practices such as cognitive behavioral therapy and motivational interviewing
- Collaborate with interdisciplinary teams to coordinate care and ensure comprehensive treatment for patients
- Maintain accurate and up-to-date medical documentation in accordance with regulatory standards
- Participate in clinical research projects to contribute to the advancement of social work practices
- Knowledge of medical terminology to effectively communicate with healthcare professionals
- Strong assessment skills to evaluate patient needs and develop appropriate treatment plans
- Experience in conducting diagnostic evaluations to identify mental health conditions
- Familiarity with cognitive behavioral therapy techniques for behavior modification and emotional regulation
- Ability to engage patients using motivational interviewing techniques to promote positive change
- Understanding of social work principles and ethics to guide professional practice
- Excellent written and verbal communication skills for effective collaboration with interdisciplinary teams
- Attention to detail in maintaining accurate and thorough medical documentation
