Role Description
This is a full-time role for a Social Worker at SourcePoint - Thrive After 55 located in Delaware, OH. The Social Worker will be responsible for providing case management, referrals, and community support services to adults over 55. Tasks may include conducting assessments, developing care plans, and coordinating services.
