Job Summary
To provide case management and care coordination services to individuals and families within the Rhode Island Works program, ensuring their compliance for specific component areas and assisting with resolving barriers to progress.
Work Schedule Demands
The following responsibilities are not meant to be all-inclusive and may be expanded or adjusted to meet component needs.
REQUIRED QUALIFICATIONS:
Has access to program case-management records. Maintains the confidentiality of all materials in accordance with State and Federal laws and CCAP policies and procedures.
All staff are expected to demonstrate cybersecurity awareness and adhere to best practices. This includes familiarity with phishing emails, creating strong passwords, and participating in regular cybersecurity training sessions.
Physical Effort/Environment
The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job successfully. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. While performing the duties of this job, the employee is regularly required to sit and talk and to hear. The employee frequently is required to use hands, fingers, handle or feel objects, tools, or controls, and reach with hands and arms. The employee is occasionally required to stand and walk. The employee must occasionally lift and/or move up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The environment is a clinical business office environment. Potential exposure to blood-borne pathogens, respiratory diseases, other infectious diseases, and other biohazardous material may require universal standard precautions. This position requires the individual to have their own transportation, a current unrestricted license to drive, and documented auto insurance, and the individual must be willing to work in schools, clients’ homes, and the community.

Job Summary

- Full-time; 35 hours/week.
- Due to the nature of the job, schedule flexibility is required to accommodate the client population.
- Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may include evening hours and hours beyond a regular workweek.
- May be required to work at any CCAP location.
REQUIRED QUALIFICATIONS:
- BSW/BA and a minimum of 2 years related experience.
- Must have demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health, and substance abuse systems; and knowledge of community resources.
- Valid Driver’s License and proof of liability insurance
- Transportation
- Must have excellent oral and writing skills.
- Provide/maintain all required immunizations and/or vaccinations
- Complete all required background checks
- Bilingual in Spanish and/or Creole; preferred
- To conduct comprehensive assessments, including assessing risks such as basic human needs, vocational/employment factors, mental health/substance use, and other related risks) on assigned clients.
- To develop a service plan with clients per the specific program's provisions.
- To provide community and/or home visitation to assigned clients.
- To provide instruction in adult daily living skills as needed and as related to barriers to academic/vocational/employment goals.
- To assist with completing emergency funding, housing, utility assistance, medical and state benefits, and other appropriate applications for assigned clients as needed.
- To maintain required client contact as the program and DHS deem sufficient.
- Maintain current and accurate record keeping for client files following the standards of DHS.
- To utilize computer database systems to maintain appropriate information on clients per program and agency requirements.
- To complete all paperwork associated with maintaining current client statistics.
- Ensure agency standards of care are met and in compliance with funders and others as required.
- To complete all paperwork required for data generation on time.
- Collaborate with other CCAP programs and administration to ensure coordination of efforts and resources and the smooth implementation of services.
- Attend program-related meetings as appropriate.
- Attend and participate in all CCAP public relations activities as appropriate.
- Attend training and development sessions.
- Other duties as assigned.
