Responsibilities Social Services:
- planning and implementing a program that offers counseling and other support services for residents and their families
- evaluating, determining and creating a plan base on the medical-related social needs of the residents
- coordinating and implementing family meetings for residents to keep the communicatin flowing
- acting as the liasion between all parties
- coordinating discharge planning
- High School Diploma or equivalent
- Previous social services experience in LTC/SNF
- A leader
- Serious applicants
- Interest in the geriatric population
