Job Responsibilities:
- Complete and coordinate all applicable clinical assessments and clinical evaluations
- Develop and review on an ongoing basis each member’s AFC plan of care
- Training, evaluating, and supervising AFC caregivers in conjunction with Program Director;
- reviewing the physician summary
- Monitoring the health status of all clients to ensure that all needed care is properly delivered
- reporting changes in the health status of any member to the client’s physician
- Provide individualized on-site nursing care to clients in the comfort of their home
- Use a comprehensive and client individualized Plan of Care with innovative approach.
- Critical thinking skills
- Clinical assessment Knowledge
- Patience and Compassion
- Communication skills
- LPN license (pass NCLEX-PN exam)
- At least two years of previous nursing experience
- Bilingual (Spanish or Haitian Creole) is a plus
