Fortcom is currently looking to hire Full-Time LPN - Nurses for Boston, Dorchester, Roxbury, Brockton, Lynn, Lowell, New Bedford, Worcester, Brockton areas of Massachusetts to perform visits to clients of our Critical Skilled Nursing Program.

Job Responsibilities:

• Complete and coordinate all applicable clinical assessments and clinical evaluations
• Develop and review on an ongoing basis each member’s AFC plan of care
• Training, evaluating, and supervising AFC caregivers in conjunction with Program Director;
• reviewing the physician summary
• Monitoring the health status of all clients to ensure that all needed care is properly delivered
• reporting changes in the health status of any member to the client’s physician
• Provide individualized on-site nursing care to clients in the comfort of their home
• Use a comprehensive and client individualized Plan of Care with innovative approach.

Education/Experience:

• LPN license (pass NCLEX-PN exam)
• At least two years of previous nursing experience
• Bilingual (Spanish or Haitian Creole) is a plus
  
  
  

Salary/Rate: Negotiable Enhance Salary starting from $65,000 per year plus benefits and 401k Retirement Plan
