Scheduler Responsibilities:

• Coordination of home-care services for a multi-disciplinary team of clinicians.
• Schedule and/or reschedule intake assessments, initial and follow up visits.
• Ensure timely completion of provider visits given each patient's condition and care plan.
• Ensure timely service provision in adherence to Medicare/Medicaid Standards of Care and State Regulations.
• Maintain a growth-oriented and service-forward mindset.
• Promote a nurturing and pro-social environment for staff and patients that embodies the highest standards of patient care (our promise to care).
  
  
  

Scheduler Qualifications:

• Strong multi-tasking, staffing and logistics skills required.
• Excellent customer service skills and service-oriented attitude required.
• Minimum 2-year customer service experience required.
• Minimum 1-year clerical experience required.
• Previous scheduling - especially medical scheduling - experience preferred.
• Familiarity with medical terminology / medical coding experience preferred.
• Proven ability to work in an interdisciplinary setting.
• Proven ability to receive training and constructive feedback.
• Excellent communication and coordination skills.
• Self-direction with the ability to work with minimal supervision.
• Ability to learn / navigate electronic documentation systems (HCHB or others) and other computer programs (MS Office).
• Bilingual (Spanish) a plus***
