What You Must Have:
- High School Diploma or equivalent
- Excellent communication and relationship building skills
- Proven experience in healthcare field or community outreach role
- Compassionate and empathetic approach to patient care
- Ability to work independently and collaboratively within a team
- Experience with EMR software

What You Will Do: :
- Initiate contact with potential clients via phone, email, and in person meetings
- Educate potential clients about palliative services available to them through InHome Connect
- Follow up and maintain an up-to-date record of interactions
- Navigate EMR efficiently to accurately record and communicate the needs and activities of clients and/or patients
- Effectively communicates patient needs to the palliative team
- Schedules patients for visits / phone calls with appropriate team members
- Acquires consent needed to enroll new patients in palliative care
- Provides support to active patients and their families – fostering open communication and support
- Performance of other duties as required
- Ensure compliance with all relevant healthcare regulations and standards
