We are seeking a dedicated Licensed Practical Nurse (LPN) for home health care in Grand Junction, CO.

• Provide personalized, safe, and high-quality 1:1 patient care in the home setting, supporting clients and their families.
• Monitor vital signs, administer medications, and perform assessments per care plans.
• Maintain infection control, document care accurately, and coordinate supplies and equipment.
• Collaborate with interdisciplinary teams and ensure client dignity and independence.
• Qualifications include an active LPN license, CPR certification, and preferred experience in home health or medication administration.
