CCTC is currently looking for a Care Coordinator to work in the BHS department. The Care Coordinator will be responsible for supporting and following through with treatment team recommendations in a fast-paced environment and meet the needs of the children ages 2.5-21 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.
Responsibilities include;
Requirements
Bachelor’s degree in Social Work, Social Science, or a Bachelor of Arts Degree from an accredited educational institution or bachelor of Science in Nursing or equivalent degree.
Minimum one (1) year case management, counseling or direct care mental health experience with children and families.
Strong interpersonal skills.
Covid-19 Vaccination required.
Responsibilities include;
- Providing interagency assessment and understanding of child’s medical, family, social, physical and educational history
- Developing interagency service planning based on the strengths of the child and family and assist in developing any activities which are necessary to help the child and family in the home and community
- Supporting network building for the child, utilizing a system’s approach to include the child’s family and community
- Developing aggressive and creative strategies to assist the child and family gain access to resources and required services identified in the treatment or service plan
- Identifying, linking, accessing, coordinating and monitoring needed services to keep the child stable in home, school and community
- Implementing and following through with psychological or psychiatric recommendations
- Monitoring the effectiveness of services to ensure continuity of care in the mental Health, behavioral and educational system
- Networking with other child-service systems
- Acting as a child and system advocate
- Providing services with cultural sensitivity
- Maintaining open communication with team members and supervisor on caseload status
- Ability to prioritize multiple tasks and meet paperwork deadlines
Requirements
