CommuniCare Health Services is currently seeking a health care professional with case management experience for a full time Multi-site Case Manager position for multiple facilities in West Virginia. This position works out of our Morgantown, WV office.

The Multi-site Case Manager job duties include:

Job Responsibilities
The Multi-site Case Manager job duties include:
- Case management
- Precerts, authorizations, and concurrent reviews
- Management of utilization, therapy minutes, insurance requirements, etc
- Coordination and communication with insurance companies.
- Working with the IDT team

Qualifications
POSITION REQUIREMENTS
- Previous Case Management experience required.
- Nursing license preferred but not required.
- Experience working in Long Term Care
- Experience working with Managed Care Services, preferably in a Long Term Care environment
- Customer service focused
- Professional and polished over the phone
