Key Responsibilities
- - Conduct in-home consultations and support families in identifying care needs
- Develop and maintain individualized care plans
- Perform quality assurance home visits and monitor client satisfactio
- Build and maintain relationships with referral sources (e.g., physicians, senior living communities, case managers)
- Represent HomeWell at community events and outreach efforts
- Support onboarding of new clients and caregiver introductions
- Collaborate with scheduling and recruiting teams as needed
- Bachelor’s degree or equivalent work experience
- 2+ years in client-facing roles, ideally in care management, hospitality, or healthcare
- Strong organizational skills, customer service mindset, and professional communication
- Experience in home care, home health, hospice, or assisted living is highly valued, but not required
- Proficient in Microsoft Office applications, including Outlook, Teams, PowerPoint, and Word
- Ability to work effectively in a high-stress, fast-paced environment with shifting priorities
- Valid driver’s license and reliable transportation required
- Base salary: $50,000 – $60,000 - depending on experience
- Performance-based bonus and incentive structure
- Mileage reimbursement and structured onboarding
