The purpose of this position is to participate in nursing care by planning, coordinating, providing and evaluating care for clients in their homes, including administering medications, doing treatments, assessing clients, communicating with physicians and families, directing direct care staff for the client’s well-being.

Leadership & Supervision:
- Oversee the daily clinical operations of the home health program, ensuring efficient scheduling and delivery of patient care.
- Provide direct supervision, guidance, and support to nursing and therapy staff.
- Ensure proper implementation of individualized patient care plans, modifying them as necessary based on patient progress.
- Monitor patient outcomes and work closely with the interdisciplinary team to ensure optimal care delivery.
- Act as a clinical resource for staff, assisting with complex cases and decision-making.
- Coordinate care services between nurses, therapists, aides, and physicians to ensure seamless patient care in the home.
- Review and approve patient admissions, ensuring that services align with medical necessity and regulatory guidelines.
- Conduct case conferences and interdisciplinary team meetings to assess and adjust patient care plans.
- Ensure patients and families receive education regarding care plans, disease management, and self-care strategies.
- Ensure compliance with state, federal, and accrediting body regulations, including Medicare and Medicaid guidelines.
- Implement and oversee quality assurance initiatives to improve patient outcomes and meet agency performance benchmarks.
- Conduct audits and chart reviews to ensure documentation accuracy and compliance with agency policies.
- Investigate patient complaints or incidents, implementing corrective actions as needed.
- Assist in recruiting, hiring, and onboarding clinical staff, ensuring proper orientation and training.
- Provide ongoing education and competency evaluations to ensure staff maintain high standards of care.
- Promote a culture of teamwork, accountability, and professional development among staff.
- Monitor productivity, staffing levels, and resource utilization to maintain operational efficiency.
- Ensure accurate and timely documentation for billing and reimbursement purposes.
- Participate in budgeting and financial planning to optimize home health services while maintaining quality patient care.
- Active RN license in [State] required; BSN or MSN preferred.
- Minimum of 3-5 years of clinical experience in home health, with at least 2 years in a leadership or supervisory role.
- Strong knowledge of Medicare home health regulations, OASIS, and electronic medical records (EMR) systems.
- Exceptional leadership, communication, and organizational skills.
- Ability to multitask and manage a dynamic clinical environment while ensuring high-quality patient care.
- Experience with home health accreditation processes (e.g., Joint Commission, CHAP).
- Proficiency in home health EMR systems such as Homecare Homebase, WellSky, or similar platforms.
- Ability to analyze clinical and operational data to drive performance improvements.
- Strong problem-solving skills with the ability to make timely, evidence-based decisions.
- Requires frequent travel to patient homes and clinical sites.
- Must be able to lift, push, pull, and transport medical equipment and supplies as needed up to 50 lbs.
- Ability to work flexible hours, including occasional evenings and weekends, as required for patient care needs.
- Able to work in a team setting and independently as well.
- Must be comfortable working with diverse patient populations, including those with chronic illnesses or disabilities.
- Graduate from an accredited professional school of nursing.
- A valid and current RN License in good standing from the Virginia State Board of Nursing.
