Position Summary
The Case Management Specialist for the Inpatient Rehab Unit functions as a member of the case management team assigned to coordinate the care and services for assigned IRC patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payers. The Case Management Specialist utilizes communication, organizational, and problem-solving skills to implement the processes of admission and discharge of patients from the IRC as directed by the RN Case Managers. The Case Management Specialist promotes and evaluates the effective utilization of resources using current knowledge and awareness of community resources. The position assumes a pivotal role with the patient care team to achieve optimal clinical and resource outcomes.

Qualifications
Education
- Associates degree, or the equivalent (meaning 2 additional years of relevant experience if no degree), is required.
- Bachelor's Degree in healthcare or related field, preferred.
- Minimum 3 years of experience coordinating care in a comparable healthcare environment where effective communication and administrative functions are demonstrated.
- Experience coordinating with community resources with demonstrated knowledge in applying specific criteria for accessing those resources.
- Experience with Microsoft Word and Excel.
- Expected to study and learn key CMS requirements for IRF regulatory compliance, and to functionally apply that knowledge to daily tasks.
- Must pass a competency test in the understanding of FIM ( Functional Independence Measure ) within 6 months of hire.
