We are seeking a Nurse Practitioner (PRN) for home risk assessments, welcoming new grads.
- Conduct comprehensive adult/geriatric health evaluations in patients†homes, SNFs, and community settings.
- Assess medical history, medications, social factors, and risks to improve care quality and close care gaps.
- Collaborate with physicians, case managers, and caregivers, providing follow-up and patient education.
- Flexible hours including days, evenings, nights, and weekends, with travel to patient homes in Horsehead, NY.
- Earn from $2,300 to $11,500/month based on assessments completed.
- Qualifications include a Masterâ€s or equivalent experience, current RN and NP licensure, certification (AANP, ANCC, AACCN), and minimum 1-year NP experience.
- Must have valid driverâ€s license and ability to travel. Benefits include mileage reimbursement, 401(k), and voluntary options.
