Seeking a Home Health Occupational Therapist in Morton Grove, IL for ongoing, full-time dayshift work.
Major responsibilities include ensuring quality care, utilizing evidence-based practices, participating in quality improvement, and promoting patient safety.
Collaborate with the care team, make clinical decisions, coordinate referrals, and participate in interdisciplinary conferences.
Demonstrate professional development, effective communication, and compassionate patient relationships.
Contribute to business growth through resource management and patient education, assessing patient needs and promoting self-care.
Requires IL licensure as an Occupational Therapist, BLS certification within 6 months, and a valid driver’s license.
Must have a Master’s in Occupational Therapy, with at least 1 year of clinical experience, and proficiency in home equipment and documentation.
Physical ability to travel extensively, lift up to 35 pounds, and assist with patient transfers is essential.
