Position Summary
The Intake and Referral Clinician supports the expansion and enhancement of our behavioral health programs by collaborating on patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. Responsible for helping shape and direct behavioral health services of the clinic in collaboration with other members of our staff. Performs individual patient assessment of needs, develops patient specific care plans, educates patient on resources available to them and conducts followup evaluations to determine patient outcome. Performs appropriate screenings and educational assessments as appropriate. Together with physician leadership, the LCSW will be accountable for maintaining quality integrated behavioral health programs for patients and staff. Fields and conducts patient calls as requested.

Qualifications
Education
- Master's Degree in Social Work, required
- Experience with psychosocial assessment, individual therapy, evidence-based counseling, and crisis intervention.
- Experience working in the medical social work or community mental health field and knowledge of Integrated Behavioral Health in the community healthcare system.
- Familiarity and comfort working with health-related issued, substance abuse and history of trauma
- LCSW: LIcensed Clinical Social Worker by the State of Oregon, required
- Certification through the National Association of Certified Social Workers and/or the Academy of Certified Social Workers, preferred
- Crisis Intervention Training and/or certification, preferred.
