Therapy Manager - Home Health

Salary starting at $105k, based on experience

Serving Milwaukee/Waukesha areas

Summary/Objective: Responsible for the planning, organization, direction, supervision and evaluation of all therapy services provided by the agency. Ensures therapy department compliance with federal, state, and local regulations and implementation of agency core programs. Ensures maintenance of records and reports concerning patient care. Ensures the orientation and training of therapy personnel. Manages the therapy department within budget. Follows all agency policies and procedures.

Essential Functions

  • Directs, plans, organizes, supervises, and evaluates all therapy services provided by the agency to achieve desired clinical and financial outcomes
  • Ensures the agency is in compliance with all regulatory requirements and standards
  • Ensures necessary equipment, supplies, and staffing are available for the delivery of the prescribed patient care
  • Completes joint visits with staff annually to ensure field compliance
  • Ensures each patients’ care plan process is carried out on a timely basis
  • Responsible for recruitment, training, supervision, scheduling, and personnel issues in the therapy department. Ensures the maintenance of licensure status for all therapy employees
  • Reviews the home health department budget and confers with the Clinical Director and Executive Director to manage costs.
  • Engages in QAPI functions of the agency
  • Represents the agency in various professional and community activities. Participates in marketing planning, activities, and evaluation as requested by the Clinical Director.


This position is a blended role with both supervisory and direct patient care functions. The candidate must carry a part time caseload, providing direct patient care, and complete supervisory responsibilities in the other half of the part time role as listed below:

Patient Care Responsibilities

  • Completes an initial physical evaluation of patient as prescribed by the attending physician, including an assessment of rehabilitation prognosis based on functional status.
  • Supervises physical therapy assistants.
  • Submits clear and appropriate documentation in a timely manner and documents patient education in the medical record.
  • Communicates effectively with physicians, supervisors, and coworkers in a timely manner.
  • Develops a care plan, which establishes goals based on therapeutic, preventive, and rehabilitative therapy actions. Includes the patient and the family in the planning process and adapts the plan of care as patient needs change.
  • Complies with agency policies and procedures. Promotes and maintains an agency environment in compliance with federal, state, and local regulations.
  • Counsels the patient and family in meeting related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.


Supervisory Responsibility

  • Collaborates with the Clinical Director to assist with the performance management and hiring of the employees within that department.
  • Supervises therapy staff in home as needed. Reviews co-signature data of Therapy Assistant notes.
  • Participates in the onboarding and continued education of therapy staff.


Additional Duties

  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.

Work Environment

The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs.

Required Education

  • Degree from a Baccalaureate or Master’s program in physical or occupational therapy approved by an accredited organization.
  • Current licensure in the State of Wisconsin
  • Current CPR certification.


Experience

  • One (1) year of home health care experience preferred.
  • Management experience preferred. Responsible for supervising the therapy department.


Skills, Knowledge and Abilities: Able to relate positively, effectively, and appropriately with patients, families, community members, and providers. Able to read, write, speak and understand English. Meets all health requirements, as required by law. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
