In-Patient
Responsible for providing clinical nutrition services, including nutrition assessment, modified diet formulation, self-management training and nutrition intervention to patients/residents. - Maintain high standards of existing ADA accredited diabetes program.
- Accountable for assuring diabetes instruction complies with national standards.
- In accordance with professional standards and hospital policies and procedures, responsible for conducting individualized needs assessments on persons enrolled in the diabetes and medical nutrition therapy patient education program.
- Assessment of patients and development of plans of care on all aspects of Diabetes self-management.ir medical nutrition therapy.
- Collaborating and communicating with team members.
- Contributing to and participating in a continuous quality improvement process to measure diabetes education program and to identify and address opportunities for improvement.
- Provide diabetes education and follow up to newly diagnosed and established adults with diabetes
- Education and follow up to new insulin pump patients
- Education and management of pregnant patients with Type I diabetes
Education:
- Bachelor’s degree in nutrition and Dietetics or Registered Nurse. Language skills, Mathematical skills, Reasoning Ability.
- Washington Dietitian Certification and National CDR Registered Dietitian (Commission on Dietetic Registration), Registered Nurse
