- Provides integrated behavioral health and substance use services within primary care, supporting prevention, intervention, and recovery.
- Conducts screenings, assessments, and crisis interventions, applying evidence-based practices tailored to diverse patient needs.
- Develops and modifies treatment plans, evaluates progress, and provides appropriate referrals.
- Collaborates with medical providers and staff to ensure coordinated care, documenting services per policies and standards.
- Utilizes Electronic Health Records, maintains licensure, and commits to ongoing education.
- Requires a Master’s Degree in Social Work, Counseling, PsyD, or PhD, Maryland licensure, and preferably two years of outpatient mental health/substance use experience in a primary care setting.
- Must demonstrate cultural competence, adaptability, and ability to serve diverse populations, including multilingual capabilities preferred.
