Coho Integrated Solutions is seeking a Health and Wellness Advocate to join our team in support of the NC Air National Guard. The NCNG IBHS is driven by a state-wide 24/7/365 toll free Helpline and serves as the portal of entry for all support services. This portal of entry must be monitored and responded to immediately by the Licensed Behavioral Health Psychotherapists on a rotating basis.
There are various locations around the state of North Carolina for this position, including but not limited to Raleigh, Charlotte, Fayetteville, High Point, Wilmington.

Responsibilities (include but are not limited to):
- Provide non-clinical case management for eligible members with behavioral health and substance use disorders whereby care is coordinated and/or integrated across all elements of complex healthcare systems (e.g., the VA, subspecialty care, hospitals, local providers), patient's community (e.g., family, public and private community-based services) and internal systems such as the Integrated Behavioral Health System (IBHS).
- Provide non-clinical care to eligible members, facilitated by registries, information technology, health information exchange, and other means to assure that eligible members get the indicated care when and where they need it in a culturally and linguistically appropriate manner.
- Coordinate non-clinical resource referrals pertaining to housing, finance, food, shelters, utility assistance, mortgage relief, etc.
- Provide Service Members with proper internal and external support networks IAW IBHS SOP, and provide follow-up services.
- Immediately refer to the appropriate office crisis situations with mentally unstable, suicidal, high-risk populations.
- Education/License:
- Bachelor's or Master's Degree in a behavioral health science
- Experience:
- 3 years of post‐graduate experience in a social work setting if master's degree is held; 1 year within military setting.
- 5 years of post‐graduate experience in a social work setting if bachelor's degree is held; 1 year within military setting.
