This job develops, leads and implements population health initiatives in primary care; assumes responsibility for implementation and operational support of system population health strategic initiatives for assigned group(s). Such activities include quality initiatives, benchmarking, utilization management, disease management initiatives and programs as may be necessary to deliver accountable care across a population. These collective programs are intended to operationalize the process and systems to standardize care, improve quality, efficiency, cost effectiveness and transparency across the continuum of care. Works closely with the infrastructure of the hospital, region and system and interacts with other networks and important stakeholders involved in healthcare delivery.

Education

Required - Bachelor’s degree in business administration or related field.

Work Experience

Required - 5 Years Related Healthcare Experience.

Preferred - Experience in physician/hospital clinic operations, inpatient services, case management, homecare/hospice or public health.

Knowledge Skills And Abilities (KSAs)

  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals.
  • Knowledge of insurance, personnel management and budgetary processes.
  • Excellent time management skills and ability to be self directed.
  • Excellent leadership skills.

Job Duties

  • Develops and implements primary care's long and short term plans to organize and manage population health initiatives and resources.
  • Ensures efficient and effective coordination of population health activities across all primary care settings & programs.
  • Leads efforts in data collection, data reporting, & measurement of key performance indicators.
  • Develops, implements, and manages the infrastructure necessary to conduct the ambulatory operations of a population health program including financial and resource needs.
  • Effectively manages approved budget and financial targets.
  • In collaboration with other leaders, develops and implements clinical policies in support of population health.
  • Manages and develops staff.
  • Develops and implements a plan that supports a clear set of actionable and targeted change management strategies including, but not limited to: employee and physician engagement/alignment, communication, coaching, collaboration, training and education as part of the overall project plan for population health.
  • Works with stakeholders to identify performance gaps and develop and implement corrective action plans.
  • Identifies new population health projects that accomplish the goals and objectives of the program(s).
  • Performs other related duties as required.
