Position Summary: Under general supervision, the Wellness Care Coordinator is responsible for MIRRORS+ Family Wellness Coordinator conducts intake/enrollment, including GPRAs/initial assessments.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
MIRRORS+ Family Wellness Coordinator conducts intake/enrollment, including GPRAs/initial assessments; links to ECI and other supports as indicated and assists families with navigation of involved/complex systems of care; offers individual/group tobacco cessation counseling, and other resources, such as quit lines and NRT; facilitates a postpartum depression group, genogram family relationship/history group, health behaviors interventions for the woman/family members, and SBIRT/brief interventions for family members; assists with facilitating family education groups, Celebrating Families!, and CHOICES; and coordinates family-based afterschool activities; participates in scheduled trainings/meetings; participates in program data collection/submission as needed to meet project goals; maintains records and completes appropriate documentation; other duties as assigned.

Education/Experience:
Master’s degree in social work or related field (LMSW or equivalent); 2+ years’ experience providing individual/group therapeutic interventions and principles of case management for diverse PPW, children, and families in SUD treatment/social services settings; Experience implementing/practicing evidence-based practices, such as Trauma Informed Care and Motivational Interviewing, and delivery of curriculum-based education services.
Language Ability:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Math Ability:
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Computer Skills:
Must have basic working knowledge of Microsoft Office (Word, Excel, Outlook and Internet Explorer).
Certificates and Licenses:
Licensed Master Social Worker (LMSW); if driving on SMH business, must have a valid Texas Driver’s License and provide proof of current insurance.
Approval Requirements:
Position may require background check approval from HCCSCD.
