Seeking a compassionate Registered Nurse Case Manager to serve patients and families in Springfield and nearby areas.
- The RN conducts assessments, develops care plans, and provides comfort-focused nursing care, responding promptly to calls, visiting patients as needed, and handling after-hours admissions and deaths.
- Work hours are 8am-5pm, Monday through Friday. Prior experience in hospice, home health, or geriatrics is preferred.
- Responsibilities include coordinating care with physicians and staff, maintaining communication, gathering and updating patient data, and ensuring accurate documentation for compliance.
- Educates patients and caregivers on medication management, personal care, safety, and end-of-life preparations. Reports issues to the Care Manager and participates in team meetings.
- Provides support during death, shares 24/7 coverage, and performs regular evaluations of aides and LPNs. Develops understanding of hospice philosophy and pain management through ongoing learning.
