Community Solutions, Inc. is seeking a dedicated and passionate candidate to fill the role of Case Manager for Ventura Case Management located in Ventura, CA. The general case management duties will include but are not limited to intake, assessment, case record management, and discharge preparation, as well as facilitation of evidence-based curricula.
Ventura Case Management provides evidence-based case management to Ventura County Probation Clients. The program addresses the fundamental principles of evidence-based correctional practice by objectively assessing criminogenic risks, enhancing intrinsic motivation, targeting higher-risk offenders, and addressing offenders' greatest criminogenic needs.

- Perform client intake within two (2) weeks of initial referral received and enter data into CSI data systems on the day the data is obtained. Enter into funder database within five (5) days of client advancement into Services.
- Case Managers are responsible for weekly curricula facilitation including Anger Management, Batterers Intervention Programing, Seeking Safety, Parenting, Substance Abuse.
- Entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
- Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
- Schedule and conduct make-up sessions as needed.
- Documentation of group and individual sessions are to be entered into the agency data systems on the same day the information is received as well as funder database.
- Must maintain a minimum of 75% completion rate for each intervention facilitated.
- Maintain a working knowledge of and abide by the Agency’s programs, policies, procedures and code of ethics. Perform other tasks as assigned by supervisor.

Qualifications
What We Are Looking For:
- Bachelor’s degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
- Strong decision-making, organizational, and interpersonal skills.
- Previous assessment and facilitation experience preferred.
- Experience using Microsoft Office and a commitment to working as part of a team.
