Also provides assessment/evaluation for involuntary commitments within KH and admissions to KDMHH. Integral part of multidisciplinary team supporting wellness and recovery of patients with psychiatric disorders and their families QUALIFICATIONS License /Certification
Required: CA Licensed Clinical Social Worker (LCSW) or Licensed Marriage Family Therapist (LMFT) Kaweah Health approved crisis intervention training within 60 days of hire/transfer.

Experience
Preferred: Experience with assessment of involuntary/persistently mentally ill patients
JOB RESPONSIBILITIES Essential

Gathers information about the patient's social, psychological, cultural, environmental and financial
situation as part of assessment.

Counsels individuals and their families regarding psychosocial needs and problems related to their medical condition.

Responds to crisis calls from community.

Conducts psychiatric assessments within KH facilities.

Performs psychiatric assessments to determine necessity for involuntary treatment.

Intervenes in crisis situations, providing needed assessment, counseling and information to patients and their families.

Provides consultation to physicians, hospital staff and community as it relates to patient care needs.

Responds to inquiries regarding mental health services within same working day. Provides program information in a professional and thorough manner.

Develops a positive working relationship with all physicians, staff and other referral sources involved in the admission, referral and patient transfer process.

Upon receiving patient referrals, reviews medical information, and assesses patient based on relevant program admission criteria. Documents assessment on appropriate forms.

Discusses program and patient needs with patient, family, physicians and staff to obtain a complete patient profile. Involves all parties, but particularly the patient and/or family in the decision making process regarding transfer.

Communicates with Psychiatrists to make final determination regarding appropriateness of admission.

Establishes the availability of funds for the services required and completes precertification for admission. Communicates with patient and appropriate administrative and patient accounting personnel throughout the process.

Communicates with patient, family, referral source, physician, and/or others as appropriate regarding admission process and date. Provides information regarding other options if admission is not possible.

Communicates with unit staff and leadership concerning the admission date, bed needs, and special needs/equipment of patient to be admitted.

Represents and markets the program(s) to the community.

Maintains monthly statistics through referral log.

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
