The Admissions RN is a registered nurse and key team member responsible for presenting hospice services and philosophy to patients and families to facilitate appropriate admissions.
- Participates in gathering, analyzing, educating, and completing admission documentation.
- Evaluates patients for hospice suitability through medical record review and obtains family agreement for admission.
- Collaborates with team members and referral sources to support high-quality, responsive care.
- Maintains professional competence and develops effective internal and external relationships.
- Utilizes interpersonal skills to educate patients and families about hospice and palliative care.
- Requires RN licensure, relevant healthcare experience, strong communication and presentation skills, and reliable transportation.
- Preferred qualifications include a bachelorâs degree and sales or service background.
