GENERAL RESPONSIBILITIES:
The Social Health Engagement Specialist (Engagement Specialist) is responsible for conducting Health Related Social Needs (HRSN) screenings, eligibility assessments, and navigation activities for Whitney Young Health patients needing linkage to appropriate resources. Central to their role, the Engagement Specialist serves to provide health advocacy, education, and health navigation designed to improve health outcomes while providing overall health literacy to prevent the development of adverse health conditions, collaborating with external partners while serving as a bridge between the patients and the health care system.
Specific Responsibilities
MINIMUM QUALIFICATIONS:
Associate degree in Human Services or a related field. One (1) to three (3) years of experience working and engaging community partners; strong organizational skills with knowledge of Excel and data tracking. Must have a valid NYS driver’s license and the ability to travel between sites as needed.
Preferred Qualifications
Minimum of 5+ years related experience in a clinical, non-profit, or managed-care organization environment. Extensive knowledge and understanding of health equity, social drivers of health, and social care data. Experience working or living in the target community and knowledge of community resources.

Specific Responsibilities
- Health Navigation: Supporting individuals with social needs screening, referral to community-based resources, conducting and managing closed-loop referrals and service coordination, benefits enrollment, and health system navigation.
- Health Advocacy: Facilitating access to respectful, high-quality, equitable health care and connecting individuals to community-based services.
- Health Education: Providing culturally competent information to improve health, enhance informed patient decision-making, and reduce barriers to accessing services.
- Maintain timely, accurate documentation of communication, referrals, supplies, inventories, and all other related paperwork.
- Utilize the Electronic Provider Assisted Claim Entry System/Medicaid Eligibility Verification System (ePACES/MEVS) and other data sources to confirm Medicaid enrollment and Health Related Social Care (HRSN) screening status, as well as existing care team management.
- Manage member/patient consent and attestation as required throughout the engagement.
- Conduct HRSN screening using the Accountable Health Communities (AHC) screening tool to assess member HRSNs.
- Conduct eligibility assessment to determine member eligibility for Enhanced HRSN services and refer members to eligible programs and services to include Enhanced HRSN services and/or existing federal, state, and local resources.
- Demonstrate the ability to use various technology platforms to ensure successful and timely referral connections are made.
- Maintain effective communication with internal team members, community members, and partner organizations to ensure acceptance, resolution, or redirection of referral requests.
- Document progress notes and actions taken with each referral as detailed in the Network Standards and Quality Program.
- Demonstrated time management and multi-tasking skills.
- Must have a valid NYS driver’s license and the ability to travel between sites as needed.
- Ensures and fulfills the minimum CDC-endorsed Community Health Worker core consensus competencies training requirements pursuant to NYSDOH.
- Demonstrates excellence in both internal and external customer service.
- Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
- Ensures and/or remains in compliance with local, state, and federal regulations, i.e., DHHS HRSA and NYSDOH, and all accreditation standards (e.g., Joint Commission and NCQA-PCMH).
- Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.
- Completes other duties as assigned.
