- The PHP Clinician conducts patient evaluations, intake interviews, assesses presenting problems, formulates diagnoses, and recommends treatment.
- Assists with developing, implementing, and modifying treatment and continuing care plans.
- Provides case management, crisis intervention, and prepares discharge summaries.
- Coordinates with referral sources and external providers to ensure seamless patient care.
- Participates in ongoing education, supervision, and clinical reviews.
- Ensures documentation complies with standards and payer requirements using electronic records.
- Serves as a resource through education, presentations, and community consultation.
- Mentors interns and students while maintaining ethical standards and confidentiality.
- Requires a Masterâs degree in counseling, 1-2 years of supervised experience, and an active license (LICSW, LMHC, or LMFT preferred).
