Job Description

Under the supervision of the CalAIM Director, the ECM Registered Nurse manages a team of Licensed Vocational Nurses & Lead Care Managers and provides complex nursing and program consultation and technical assistance to them. The Registered Nurse works with both the LVN & LCM to plan, develop, organize, monitor, and evaluate programs and studies on the delivery of health services. The ECM RN may also serve in a lead capacity to Nurse Consultants and other health-related multidisciplinary staff; develops and evaluates program standards, policies, and procedures; and does other related work. The position will oversee staff, the day-to-day operations including provision of outreach, health and psychosocial education, linkage to care, patient advocacy, and documentation.

QUALIFICATIONS/LCENSURE:

  • Bachelor of Science degree in nursing, medical, social work, psychology, public health, or related field preferred.
  • 3-5 years’ related experience working as a Registered Nurse.
  • Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes.
  • Experience preparing and delivering presentations.
  • Experience managing a team preferred
  • Extensive knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing.
  • Strong knowledge of community health evaluation tools, approaches, frameworks, and methods; and
  • Bilingual English/Spanish preferred (read, write, speak)
  • Licensure/Certification - Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management:
  • Valid CA Registered Nurse license

RESPONSIBLITIES

  • Responsible for overall management of CalAIM Enhanced Care Management and Community Supports program staff to include day-to-day operations, support service provisions, quality measures, reporting and performance feedback.
  • Develop and grow program as needed to support eligible members and patients.
  • Oversee day-to-day operations and support service provisions.
  • Develop and engage in Multidisciplinary Team Meetings.
  • Support Clinical Consultants (LVNs, medical providers), Program Managers, Program Coordinators, Lead Care Managers, Housing Care Managers, Housing Specialists/Navigators, and other staff as needed.
  • Work with Homeless entities to support Care Plans.
  • Provide mentorship and supervision for staff.
  • Serve as point of contact for patients and staff, and collaborating agencies.
  • Coordinate all CalAIM Enhanced Care Management and Community Supports programmatic activities, including policy and procedures.
  • Create necessary infrastructure to advance program, including coordination with St. John’s providers and staff, as well as leadership and staff of partner agencies.
  • Maintain up-to-date knowledge and database of community resources and referral options for patients.
  • Ensure patient data is entered in E-Clinical Works.
  • Review and audit charts and any documentation required under the contracts and grants.
  • Supervise, coordinate, and monitor program evaluation activities.
  • Create and coordinate MOUs with all partner and referral agencies.
  • Write and edit progress reports for affiliated grants by coordinating with relevant staff to collect the necessary data to report on achievement and evaluation of contract goals and objectives
  • Create presentations, trainings, and technical assistance tools for St. John’s providers and staff.
  • Assist in the development of education/promotional materials.
  • Attend meetings, trainings , and other work-related events as needed.
  • Work with Chief Program Officer, Development Team and Chief Medical Officer to promote integration of Program.
  • Work collaboratively with other departments to identify and evaluate program opportunities according to overall strategic objectives.
  • Work with IT to ensure proper questions for intake forms to identify homeless or at risk of homeless and chronic health conditions.
  • Attend local, state and national healthcare for the homeless conferences (e.g. through NACHC).
  • Work with clinical team to integrate designated programs into clinical workflow.
  • Other duties may be assigned or may be modified as business needs dictate.
