New Vanderbilt Rehabilitation and Care Center is seeking a dedicated Social Worker to join the team. Requirements of the Social Worker: Year of active patient/resident contact in a health care setting. Must have at least 2 years if experience in a long-term care setting. Experience working with MDS, discharge planning and electronic medical records a plus. Bachelor's Degree in Human Services, Psychology or Social Work.
