Summary: Responsible for facilitating clinical services for program as outlined by facility standards and in accordance with clinical philosophy of program. The Client Navigator will support clients within Pyramid Healthcare to ensure that they have a smooth transition in the continuum of care.

Essential Duties And Responsibilities

  • Evaluates behavioral, emotional, cognitive, spiritual, and social needs of clients and family members and consults with treatment team regularly for the development of and progress towards treatment goals in accordance with clinical policy and accreditation guidelines.
  • Facilitation of individual sessions to develop treatment plans.
  • Facilitates all clinical services for clients including individual and family sessions, therapeutic groups, and referral source/ outpatient provider contact as applicable
  • Responsible for maintaining clinical record for clients with appropriate documentation of clinical services and client program engagement in ECR in accordance with program policy.
  • Provides documentation and support for utilization review in a timely way and participates in UR process as outlined in program policy.
  • Participates in aftercare plan development and outcome data process.
  • Provides case management services for clients and families through coordinating transportation needs, providing individual and family case management sessions, assisting in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs Conduct Level of Care Assessments as required.
  • Completion of all required trainings as designated by the company and accreditation/licensing entities.
  • Other duties as assigned.

Required Qualifications

Education, Licensure, & Experience

Employees Supporting CT, NJ, VA, MD, NC, GA, OH, WV Sites

  • Preferred Bachelor’s Degree in human services or related field and at least 1-2 years of experience in substance abuse and mental health field. High school diploma or equivalent considered with 1-2 years of experience in similar work.

Employees Supporting Pennsylvania Sites must meet one of the following:

  • Has an active statewide clinical license or certification by a statewide certification board.
  • A Master’s Degree in a human services area and are working towards certification by a statewide certification board (CAADC). A Bachelor’s Degree in a human services area and are working towards certification by a statewide certification board (CADC).
  • An Associate’s Degree in a human services area and working towards a certification by a statewide certification board (CAAC).
  • High School Diploma or equilivant and working towards a certification by a statewide certification board (CAAC).
  • Current licensure in this Commonwealth as a registered nurse and a degree from an accredited school of nursing and one year of qualifying clinical experience.

Physical Demands

While performing the duties of this job, the employee is frequently required to talk or hear. The employee is regularly required to sit, stand, use hands to finger, handle, or feel, and reach with hands and arms. The employee is seldom required walk, stoop, twist, kneel, or crouch. The employee must seldom lift and/ or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Work Environment

This position is onsite. The noise level at facilities and offices is usually moderately quiet & indoors.
