Description

We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our team and provide essential support to telephonic nurses in managing high-risk Medi-Cal patients. The CHW will serve as a vital link between patients, healthcare providers, and community resources, working to improve patient health outcomes and reduce health disparities. This role requires a proactive approach to in-person patient engagement and the ability to work effectively in diverse settings. The CHW will play a pivotal role in locating hard-to-reach members out in the community, including FQHCs, hospitals, food pantries, shelters, and the surrounding community.

This position will primarily take place out in the field in the Inland Empire area, with some tasks to be completed at home.

What You'll Do

  • Patient Engagement and Outreach:
    • Conduct outreach to high-risk MediCal patients in the IE or Antelope Valley, as identified by telephonic nurses.
    • Build rapport and establish trusting relationships with patients and their families.
    • Provide culturally sensitive support and communication.
    • Conduct home visits to assess patient needs and provide support.
    • Travel to various community settings, including hospitals, Federally Qualified Health Centers (FQHCs), food pantries, and homeless shelters, to locate and engage patients.
    • Schedule and conduct meetings with patients in appropriate community locations.
    • Advocate for patients' needs and preferences.
  • Care Coordination and Support:
    • Perform assessments of patients after completing in-person engagement.
    • Collaborate closely with telephonic nurses to address coordination of care needs or complex medical condition support.
    • Provide education and support to patients on disease management, medication adherence, and healthy lifestyle choices.
    • Assist patients in navigating the healthcare system, including scheduling appointments, arranging transportation, and accessing1 specialty care.
  • Resource Navigation:
    • Connect patients with community resources such as food assistance, housing support, transportation, and social services.
    • Provide information and referrals to relevant organizations and programs.
    • Assist patients in completing applications for benefits or services.
  • Data Collection and Documentation:
    • Maintain accurate and timely records of patient interactions and activities.
    • Collect data and report on patient progress and outcomes.
    • Use electronic health record systems and other data management tools as required.

Qualifications

  • High school diploma or equivalent
  • CHW Certification or willingness to obtain certificate within 90 days of employment
  • Valid California driver's license and reliable transportation
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Cultural competency and sensitivity to diverse populations
  • Knowledge of community resources in the IE or Antelope Valley
  • Basic computer skills and experience with electronic documentation
You'll be great for this role if you have:
  • Associate's degree in a health-related field
  • Experience working with MediCal populations
  • Bilingual in English and Spanish
  • Experience in care coordination or case management

Environmental Job Requirements and Working Conditions

  • This position requires significant travel within the Inland Empire, including home visits and visits to hospitals, FQHCs, food pantries, homeless shelters, and other community locations.
  • This is a full-time position. Expects hours are M-F 8:30 AM - 5 PM. Flexible work hours may be required to meet patient needs.
  • The target pay range for this role is $24.00 - $28.80 per hour. This salary range represents our national target range for this role.
