• esDevelops a comprehensive social history and assessment of residents that includes problems and strengths, special needs and preferences (social, religious and/or cultural) and how to best address and support them for the care pl
• anAssists patients/residents and their caregivers with transitioning to new environments, orienting them to the facility and our services, educating on resident rights and other aspects of transitioning to/from care facili
• tyActs as mediator to resolve concerns/conflicts that may arise regarding care and treatment, admission and discharge procedures, physician care and/or any other situations that could impact or negatively affect the patient/reside
• ntParticipates in the development of individual’s care plans including the goals and approaches for Social Servic
• esActively participates in the coordination of care services, evaluating the ongoing needs of residents (attends resident care plan meetings, rehabilitation and resident/family meetings, departmental staff meetings, annual policy review meetings, monthly quality assurance meetings, etc
