Adult Case Manager- Canton, TX

We're seeking a dedicated Adult Case Manager to join our team and support individuals living with mental health and/or substance use challenges

What You'll Do

• Provide personalized services to individuals facing challenges such as risk of harm, housing instability, unemployment, and medication non-compliance.
• Support clients through one-on-one and small group sessions, promoting recovery, independence, and personal growth.
• Deliver skills training and services in Supported Employment and Supported Housing.
• Help clients build stronger community connections, develop daily living skills, and access local resources.
• Complete assessments (ANSA, Rehab) and develop recovery plans tailored to individual needs.

Requirements

Bechelor's degree in human services field (required).

Expereince in mental health or substance use disorder preferred.
