Description

CSI is in need of a Community Transition Coordinator to join our Chase Center program located in Waterbury, CT. The program is an adult residential facility that administers assessments, monitors clients' compliance with service plan goals, and involves clients in services that address their criminogenic tendencies. Though case management, supervision, and provision of evidence-based groups, clients gain the tools and support necessary for successful community re-entry. The Community Transition Coordinator is responsible for all aspects of client discharge planning and community transition. Implements all Agency and Department of Correction policies and procedures. Provides exit packets, community-based referrals and resource linkages and collects and reports data. About CSI:

(CSI) is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved, or at risk of involvement, in the child welfare, juvenile justice, or criminal justice system. Since CSIs founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in 9 states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.

CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. CSI is committed to systematically evaluating the integration of the trauma-informed values of safety, trustworthiness, choice, collaboration, and empowerment at all levels of the organization's policies, procedures, and structures to guard against the possibility of re-traumatization.

  • Maintain all aspects of client discharge planning, including the following duties:
  • Providing and completing client discharge plans for the Department of Corrections
  • Coordinating all exit requirements with Probation and Parole Officers
  • Participates with Prisoner Reentry Councils and Forums and follows up on resource leads
  • Referring offenders to community-based services and resources
  • Following up to ensure follow through with referrals
  • Collecting and reporting data on services and performance metrics
  • Collaborating in regular performance management meetings
  • Collaborating with internal CTC team for regular case review and supervision
  • Conducting Post Release Follow Up
  • Reporting Post Release Data
  • Completes client paperwork via Case Management electronic system
  • Abides by agency confidentiality policies and applicable confidentiality statutes and regulations
  • Maintains safety, appearance and cleanliness of CSI office and facility spaces
  • Adheres to program procedures and enforces rules, regulation and policies
  • Responds to client and program crises and reports incidents to supervisor and program as detailed in Community Solutions, Inc. reporting policies and procedures
  • Attends and participates in staff meetings, supervision, and on-going agency training

Schedule: Sunday/Monday/Tuesday 9am-5pm and Wednesday/Thursday 12pm-8pm

Hourly Rate: $22.10

Qualifications

Qualifications

  • Associates Degree in a social science field and one year experience in social services or similar field. Education may be substituted with experience on a year for year basis.
  • Excellent oral, written and interpersonal skills desired.
  • Experience with Microsoft Office.
  • Valid drivers license and transportation
