Join a leading home health and hospice provider across seven states, dedicated to delivering compassionate, patient-centered care aligned with core values of CARES.

• Evaluate patients' physical conditions and develop personalized treatment plans.
• Implement therapeutic exercises to enhance mobility and reduce pain.
• Educate patients and caregivers on safety and self-care strategies.
• Collaborate with a multidisciplinary team to ensure holistic care.

Qualifications include an active Physical Therapist license, Doctorate or Master’s degree, CPR certification, reliable transportation, and preferred minimum one year of experience.
