The Wellness Coach provides support to both patients and the care team in helping to facilitate and empower patients to engage in their own health.

Utilizing the risk stratification tool for overall health status, this role is primarily working with patients served by every clinic that have scores ranging from healthy through low-rising risk with an emphasis on prevention-oriented work.

This role will be based at our offices within our Center for Learning and Innovation and the Wellness Coach will travel to our clinics as needed.

Accountabilities Include

  • Works with the Wellness Program manager to create and/or facilitate The Learning Well offerings.
  • Facilitates or co-facilitates educational classes on health promotion, prevention, and wellness according to best practice facilitation standards.
  • Facilitates group sessions in Spanish.
  • Supports providers and others who offer wellness groups or classes with planning, facilitation, and patient follow up, as it relates to health promotion and components of the wellness wheel.
  • Works as part of a team in program planning, implementation, and evaluation.
  • Collaborates with patients and care teams to provide support and increase knowledge and understanding of health conditions as well as provide education regarding preventative care measures in a group setting
  • Documents patient care in our electronic medical record system and is responsible for following workflows that result in capturing activities to meet patient engagement standards as outlined in the APCM program.
  • Supports focus on wellness promotion utilizing the Wellness Wheel to engage and support patients in identifying and working toward their own personal wellness goals, in collaboration with the primary care team.
  • Sustains connections with patients through follow-up phone calls, and all HIPAA compliant patient communication platforms and with individual and/or group coaching sessions.
  • Utilizes motivational interviewing to assess patient readiness to change.
  • Utilizes a trauma informed care practice of engagement with patients; for example, asking a patient, “What’s happened to you?” rather than, “What’s wrong with you?”

Required

Education and/or Experience:

  • Minimum: High School Diploma
  • Bilingual: Spanish/ English.
  • Three to five years of experience in Community Health
  • Basic knowledge of adult learning models
  • Excellent interpersonal communication and problem-solving skills
  • Skills to intervene and promote reconciliation, compromise and positive outcomes in difficult interactions.
  • Proficiency in computer and common programs such as Microsoft Outlook.
  • Achieves Community Health Worker certification requirements within 6 months of employment and maintains certification while employed.

Preferred

  • Bachelor’s degree in health education, health promotion, athletic training, nutrition, psychology, or social work.
  • International Coach Federation or similar coaching certification / recognition.
  • Experience with Epic (our electronic medical records system).
  • Experience with trauma informed care.
  • Experience with motivational interviewing.
