- The PAS Medical Records has responsibilities of Medical Records custodian including maintaining patient's records at HVC and protecting patients’ rights according to state and federal laws. Oversees productivity of HVC Medical Records including the supervising of staff.
- Supervises and contributes to evaluations of all employees in their assigned area.
- Sends second requests to companies for copying records.
- Purges charts as needed. Makes "B" charts as needed.
- Prepares charts for audits. If necessary, copies and sends these records to requesting entities.
- Prepares all handwritten testing reports and dictation tapes for transcription.
- Receives all death certificates, has them signed by physician and returns them to funeral home.
- Researches obituaries in local newspapers, determines if HVC patient, and if so, enters in computer and removes chart to deceased files.
- Prepares "dismissed" charts, mails letter to patient and enters effective dismissed date in computer.
- Is responsible as the Medical Records Custodian
- Strives to constantly improve processes in assigned areas.
- Coordinates schedules among all assigned staff including approving time off requests.
- Receives all hospital records and verifies if patient is established with the practice.
- Faxes results to referring physician when results cannot be sent via Imed.
- Files documents in electronic or paper charts; organizes all documentation properly in chart.
- Pulls charts for physicians, other personnel, clinics and incoming correspondence.
- Prepares legal charts for director's approval. Sends to copy service or copies and mails in timely manner.
Education And Work Experience
- High school graduate.
- Minimum 3 years’ experience in medical records field.
- Basic computer skills.
- Ability to read, write and speak intelligible English; ability to communicate well with patient, patient's family, peers and medical personnel.
- Must be able to apply good judgment when making decisions, and have advanced prioritization skills and basic mathematical skills.
- Work involves a variety of physical activities including sitting, standing and walking. Bending, stooping and light duty lifting are required.
- Must be able to sit for long periods of time and utilize a computer and office productivity machinery.
- Must have normal hearing or utilize adaptive hearing devices. May require off-site travel.
