Job Summary:

The Community Alignment Specialist (CAS) will participate in the development of community resources in partnership with both public and private agencies within the community, and support the development and implementation of a marketing plan for the community. This position will foster relationships among civic, community and health and human services partners and will support the evolution of plan programs, services and supports that are designed to meet the needs of families with newborn children. The CAS will coordinate closely with contractors supporting the implementation of the project.

The CAS will support the development of systems to solicit and document feedback from home visiting nurses and the Family Connects Clinical Manager regarding unmet needs in the community, and, with the support of the team, plan toward closing those gaps as possible.

In addition, the CAS will support the marketing and public relations efforts of the Family Connects program. The CAS will also participate in establishing and maintaining communication between community resources, physicians, nursing personnel and other health-care professionals regarding program objectives, policies and procedures. The CAS will compile and maintain records, reports and documentation of program activities regarding community relations for use in program evaluation, and will provide general support to program activities as requested.

Essential Functions

  • Establish and nurture relationships, with community resources and services that provide support to families in a timely and appropriate manner as referred by Family Connects nurses and that participate in the community system of care for young children and their families.
  • Identify resources, establish relationships with providers, maintain an updated list of resources, and identify and track gaps in needed community resources so that the broader community can be induced to grow these resources over time. Attend nurse case conference meeting to provide individual-case consultation about community resources to support nurse home visitors as they connect families with community resources, as needed. Coordinate to support these functions through a contract with an external information resource and referral agency, if developed for the project.
  • Market the program by communicating with referral sources (for example, ob/gyn practitioners, hospitals, pediatricians), communicating with groups that interact with families (for example, employers, newspapers, churches, childcare agencies), and soliciting support from community leaders (for example, to publicize the program and to provide incentives for families such as free diapers).
  • Increase the array of options for pregnant women and new mothers to know about and to sign up for a Family Connects home visit by work with health providers and other organizations about referrals to and support for the program.
  • Support external boards or councils supporting community alignment for the Family Connects program as it coordinates services with others in the early childhood system of care.
  • Compile and maintain records, reports, and documentation of program activities regarding community relations for use in program evaluation.

Minimum Education, Training, And Experience Required

  • Bachelor’s degree in a related field (for example, public health, psychology, social work, nursing communications, marketing) preferred.
  • Requires progressive experience in human services in the public sector, skill in interacting with human service providers and agency leaders, and an understanding of the role of evaluation in social services.
  • A combined five years of experience in a related field is desired.

  • Ability to communicate clearly and professionally with a range of community stakeholders.
  • The position requires an understanding of and experience in communication and public relations strategies and writing experience.
  • The successful candidate will possess leadership skills and a willingness to take initiative and be proactive. Ability to work independently, as well as in teams.
  • Ability to work well with diverse populations through inclusive and culturally responsive practices.
  • Experience providing administrative support to community meetings or coalitions
  • Competence with Microsoft Office suite,

Physical Requirements

Medium Work – Lifting up to 50 pounds on an infrequent basis (less than one lift every three minutes) and/or carrying up to 25 pounds while walking. Pushing or pulling up to 75 pounds infrequently or up to 115 pounds occasionally (no more than once per hour).
