EveryMind is hiring a Homeless Outreach Case Manager to join our Adult Case Management team. This role is responsible for providing outreach, engagement, assessment, and case management services to homeless individuals eighteen years old and older in Montgomery County, MD.

Primary Function

Responsible for providing outreach, engagement, assessment, and case management services to homeless individuals eighteen years old and older in Montgomery County, MD.

Essential Duties And Responsibilities

  • Maintain flexible hours to regularly canvass and perform outreach for both potential clients, active clients and community businesses. This includes regular evening and early morning availability along with support of the County PIT or other count efforts and surveys.
  • Provide or arrange transportation, as needed, to/from appointments or attending with client when necessary. This does require the use of a personal vehicle capable of daily use and accommodating a client during business hours.
  • Maintain records of client interactions, referrals, written needs assessments, dispositions, and all client related paperwork per reporting guidelines. This includes internal tracking and county electronic records. Timely documenting and tracking all client interactions, related supports, and formulating observations into written needs assessments.
  • Effectively utilize County electronic health record database to enter clinical service plans, contact notes, and intake information. This includes regularly updating client data and profile to match county reporting requirements.
  • Advocate on behalf of clients for needs, services, and supports while serving as link to and provide direct assistance with accessing resources, such as MANNA food donations, shelter, housing, financial entitlements, donation sites, DHHS, food stamps, Social Security, medical care, and all other County resources available to the client.
  • Coordinate client mental health support and ongoing care with therapists, psychiatrists, and hospitals as needed.
  • Submit reports and complies with Program Director requests for data reporting, as needed.
  • Develop case plans, monitor progress, and reassess client needs and stage of change.
  • Complete and engage in trainings as needed to support client needs and effectively perform duties.
  • Perform other duties as assigned.

Supervision to be exercised:

Occasional supervision of interns and volunteers.

Supervision to be received:

Reports to Manager, Homeless & Housing Support Services.

Qualifications

Strong Personal Resilience and Mental Health:

Given the high-stress nature of the role, it is essential that candidates possess a high level of personal resilience and maintain stable mental health. This ensures that they are well-equipped to provide effective support and maintain the professional standards required for client care.

Education:

Bachelor’s degree in social work discipline.

Experience:

At least one year of experience in providing services to low-income, homeless, psychiatric, substance involved, or co- occurring adults.

Skills:

Excellent oral and written communication skills. Must be able to organize and prioritize. Experience with Microsoft Office platforms, Electronic Health systems and accurate data entry skills. Bilingual Spanish preferred.

Abilities:

Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO, board members, clients and family members or caretakers, interns, volunteers, paid staff, and community partners.

Ability to express ideas with clarity, confidence and sensitivity. Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines. Familiarity with working and hiring interns and volunteers.

Able to supervise multiple people.

The employee must be able to lift and/or move up to 35 pounds frequently. The job's specific vision abilities include close vision, distance vision, and the ability to adjust focus.

Due to the nature of the content that is handled in the program, anyone who has a history of trauma or suicidality could be at risk of being triggered or re-traumatized. This is especially true for people who have experienced these issues within the past 12 months and therefore we may not recommend this work if their experience is so recent.

Clearance Requirements:

The employee must pass a criminal background check. Certain job functions may require employee to pass a child sexual abuse and other criminal activity as well as a credit check. All staff providing services to staff or clients, must be free of illegal drugs and abuse of alcohol, and must be fully able to always perform all required duties.

The employee must be available to perform the duties of the job during most, but not limited to core business hours (between 9-5pm, M-F), some flexibility with schedule may be available (evening and/or weekend hours). Employees will work mostly on-site but may work some hours remotely with the approval of supervisor.

The employee is also required to travel to different locations. Therefore, the employee must have access to reliable transportation.
