About The Role

Join our compassionate team at Nassau Rehabilitation and Nursing Center as a Home Health Aide Escort. In this important role, you will provide essential support to our residents by escorting them safely to and from clinic and dialysis appointments. This position offers meaningful work with a consistent morning schedule and the opportunity to make a positive difference in residents’ lives every day.

Key Responsibilities

  • Escort residents safely to and from clinic and dialysis appointments.
  • Arrive one hour prior to scheduled clinic appointments.
  • Maintain a full nursing uniform and wear your ID badge at all times, both in the facility and at the clinic.
  • Assist residents in preparing for their appointments.
  • Ensure residents receive appropriate out-on-pass snacks.
  • Monitor residents closely to prevent falls and altercations, intervening immediately as needed.
  • Report all incidents or accidents promptly to the Clinical Coordinator or Assistant Director of Nursing Services (ADNS).
  • Obtain reports from clinic visits and ensure residents have all necessary paperwork and supplies.
  • Be knowledgeable of residents’ DNR/DNI status, ensuring all non-hospital DNR forms accompany residents.
  • Advocate proactively for residents during clinic visits, communicating any challenges back to the facility.
  • Represent Nassau Rehabilitation and Nursing Center professionally at all times, adhering to policies, procedures, and abuse laws on and off facility property.
  • May assist with additional clinic visits or unit staff duties as needed.


Requirements

  • Current New York State Home Health Aide (HHA) certification required.
  • Ability to perform all tasks and duties of a Home Health Aide.
  • Strong communication skills: ability to read, write, speak, and understand English.
  • Knowledge of age-appropriate care for residents.
