Key Responsibilities
- Escort residents safely to and from clinic and dialysis appointments.
- Arrive one hour prior to scheduled clinic appointments.
- Maintain a full nursing uniform and wear your ID badge at all times, both in the facility and at the clinic.
- Assist residents in preparing for their appointments.
- Ensure residents receive appropriate out-on-pass snacks.
- Monitor residents closely to prevent falls and altercations, intervening immediately as needed.
- Report all incidents or accidents promptly to the Clinical Coordinator or Assistant Director of Nursing Services (ADNS).
- Obtain reports from clinic visits and ensure residents have all necessary paperwork and supplies.
- Be knowledgeable of residents’ DNR/DNI status, ensuring all non-hospital DNR forms accompany residents.
- Advocate proactively for residents during clinic visits, communicating any challenges back to the facility.
- Represent Nassau Rehabilitation and Nursing Center professionally at all times, adhering to policies, procedures, and abuse laws on and off facility property.
- May assist with additional clinic visits or unit staff duties as needed.
- Current New York State Home Health Aide (HHA) certification required.
- Ability to perform all tasks and duties of a Home Health Aide.
- Strong communication skills: ability to read, write, speak, and understand English.
- Knowledge of age-appropriate care for residents.
